Serious One Car Motor Vehicle Accident

A serious one vehicle MVA was toned out to the Smithtown Fire Department at 11:23:35 AM, on Tuesday, January 13, 2009  at 7 Route 25A and Summerset Drive across from Avalon Garden.

 

Responding to the alarm were Chief’s 4-2-33 (Third Assistant Chief Sisino), 4-2-31 (First Assistant Chief Duffy) and 4-2-30 (Chief of Department Duckham), Engine 4-2-5, Engine 4-2-1, Rescue 4-2-9 and Ambulance 4-2-62.

 

Upon arrival Chief Sisino witnessed a vehicle that struck a tree, caught fire and had one victim entrapped. Suffolk County Police were on the scene first and began battling the fire with extinguishers.

 

Jeff Bressler, Public Information Officer for the Smithtown Fire Department commended PD’s early response, “The Smithtown Fire Department commends PD for their swift response to the fire until we arrived. Without protective gear they took immediate action to protect the trapped driver.”

 

Arriving on the scene the Smithtown FD quickly went into action extinguishing the fire quickly so they could begin the extrication of the entrapped driver.

 

“I am proud of the swift and professional way our firefighter extinguished the blaze and immediately commenced with the extrication process. We initially removed the passenger door to get to the injured driver. In order to remove him we had to take out the rear seat of the vehicle. The operation was preformed seamlessly.”

 

Once removed the drive was airlifted by Suffolk PD aviation. The helicopter took the unidentified driver to the Stony Brook University Medical Center for treatment.

Overnight MVA

The Smithtown Fire Department toned out a 16/23 (Ambulance requested – Motor Vehicle Accident/ Heavy Rescue) at 01:17:48 (1:17:48 AM) on Saturday, November 15th, at Meadow Road and Springbriar Lane. Upon arrival Second Assistant Chief Hansen (4-2-32) witnessed a white Toyota that had struck a utility pole with the driver as the only occupant.  Working with Chief Hanson at the scene were Third Assistant Chief Sisino (4-2-33) Engines 4-2-4 and 4-2-15, Rescue 4-2-9 and Ambulance 4-2-62.

The driver of the vehicle, a 17 year old from Kings Park, was pinned in the driver’s seat.  A complex extrication was undertaken that involved the removal of the passenger door, vehicle’s roof and dash board assembly.

The injured driver was removed and transported to the Stony Brook University Medical Center. No report on his injuries at this time.

Main Street MVA

The Smithtown Fire Department toned out a 16/23 (Ambulance requested – Motor Vehicle Accident) at 17:56:13 (5:56 PM)  on Friday, November 14th to Main Street and Route 111. Upon arrival Third Assistant Chief Sisino (4-2-33) witnessed a two car collision. Upon arrival he positioned Rescue 4-2-9 and ambulance 4-2-62 at the scene. One injured driver was transported to St, Catherine of Sienna Medical Center. The other driver in the incident did not require medical attention.  All units were up upon the completion of a road clean-up.

Roll Over

The Smithtown Fire Department responded on Tuesday evening to a signal 23 MVA  with a roll over on Town Line Road near Route 111. Fortunately no serious injuries resulted from the two car accident.

Afternoon MVA

The Smithtown Fire Department toned out a 16/23, ambulance requested, motor vehicle accident on Wednesday, August 20 at 12:38:48 at Veterans Memorial Highway and Northfield Road. The incident involved a car striking the rear of a telephone company boom truck with a third vehicle also making contact with the truck.

The Smithtown Fire Department working in conjunction with Suffolk County Police Emergency Services preformed an extrication to remove the female occupant of the car.

Smithtown worked the scene with Chiefs 4-2-30, 4-2-31 and 4-2-33, Engine 4-2-15, Rescue 4-2-9 and Ambulances 4-2-62 and 4-2-64.

After extrication, the driver was removed and transported via helicopter to the Stony Brook University Medical Center.  The Hauppauge Fire Department provided Engine support for the helicopter landing.

Condition of the injured is unknown at this time. The Suffolk County Police Department is investigating the cause of the accident.

Early Morning MVA

The Smithtown Fire Department toned out a 16/23, ambulance requested, motor vehicle accident on Saturday morning August 9th at 04:11:15. Upon arrival Chief of Department Thomas Duckham, 4-2-30, witnessed a white Ford Taurus that had struck a tree in front of 8 Thatch Pond Road north of Edgewood Avenue.

Initially there were three injured occupants found at the scene with a report that additional occupants had fled the area.

Smithtown responded with ambulances 4-2-64 and 4-2-62, Engines 4-2-15 and 4-2-4, and Rescue 4-2-9. Additional Chiefs at the scene were 4-2-31 and 4-2-33. The Kings Park Fire Department provided a mutual aid ambulance.

Three of the injured were taken to the Stony Brook University Medical Center for treatment. Two additional injured were located away from the car and transported to St. Catherine of Sienna Hospital by the St. James Fire Department.

Condition of the injured is unknown at this time. The Suffolk County Police Department is investigating the cause of the accident.

MVA –ROUTE 25A

A signal 16/23, ambulance requested, MVA with an ejection was toned out to the Smithtown Fire Department at 00:32 (12:32am) on Thursday, July 03, 2008. The incident was reported eastbound on Route 25A across from Avalon Gardens.

 

Responding to the scene were Chiefs 4-2-30, 4-2-31, 4-2-33, Rescue 4-2-9, Engine 4-2-15 and Ambulances 4-2-61 and 4-2-62, EMS 4-2-81 and an ambulance from the Nesconset Fire Department.

 

Upon arrival units witnessed a one car accident with vehicle and foliage debris spread over a significant area. Two occupants of the vehicle were counted.

 

Rescue 4-2-9 preformed a preliminary search in a ravine off the side of 25A to determine if there were additional occupants in the vehicle. The initial search was negative.

 

Two injuries required the need for transport. 4-2-62 took one occupant to St. Catherine of Sienna Hospital. The Nesconset FD ambulance transported the other occupant to the Stop and Shop parking lot on Jericho Turnpike in Smithtown where a Suffolk County Police Helicopter and Engine 4-2-1, on watch, were waiting to airlift the injured to the Stony Brook University Medical Center.

 

 There is no report on the condition of either occupant at this time.

 

The Suffolk County Police Department remained at the scene to determine the cause of the accident. K-9 assistance was also requested.

MVA – Route’s 347 and 111

At 2:26 (2:26am) on June 28, the Smithtown Fire Department toned out a 16/23 Ambulance requested/Motor Vehicle Accident for a two car accident at the intersection of Route’s 111 and 347.

Upon arrival EMS 4-2-80 viewed a two car accident involving a Jeep Grand Cherokee and a taxi cab. Each vehicle had two occupants. The driver of the Jeep was pinned and required heavy rescue to release him from the vehicle.

Chief 4-2-33 (Sisino) was in command. Responding to the alarm were Engines 4-2-1 and 4-2-4, Rescue 4-2-9, Ambulances 4-2-61 and 4-2-64 and a mutual aid ambulance from CI-Hauppauge.

Rescue 4-2-9 gained entry to the entrapped driver. Ambulance’s 4-2-61 and 6-2-64 transported patients to the Stony Brook University Medical Center while CI – Hauppauge transported to St. Catherine of Sienna.

This call was the Smithtown Fire Department’s 1600th alarm of 2008.

Motor Vehicle Accident – Heavy Rescue

A signal 16/23, ambulance requested, MVA, heavy rescue was toned out to the Smithtown Fire Department at 19:56 (7:56 PM) on Tuesday, May 20, 2008. The incident was reported eastbound on Jericho Turnpike between Plymouth Blvd. and Meadow Road.

 

Responding to the scene were Chiefs 4-2-30, 4-2-31, 4-2-32, 4-2-33, Rescue 4-2-9, Engine 4-2-15 and Ambulances 4-2-61 and 4-2-62.

 

Upon arrival units started working on a two car head on collision involving a driver in each vehicle. One driver was able to leave the vehicle under his own power.

 

In order to remove the driver of the other vehicle, 4-2-30 requested heavy rescue. Spreader and a cutting tool were used to force the driver’s side door so the occupant could be removed.

 

Two injuries required the need for transport. 4-2-61 went to St. Catherine of Sienna Hospital where the driver of one of the vehicles sustained minor injuries.

 

Ambulance 4-2-62 transported the other driver to the Stop and Shop parking lot on Jericho Turnpike in Smithtown where a Suffolk County Police Helicopter was waiting to airlift the driver to the Stony Brook University Medical Center. There is no report on her condition at this time.

 

UPDATE: May 21, 2008 – 18:17

A fatality resulted in the two car collision. Mary Stone, 63, of Coram was pronounced dead after she was airlifted to Stony Brook University Hospital.

Amr Mohamed Nagy, 18, of Commack, who was transported to St. Catherine of Sienna, was treated and released.

MVA – Sunday, April 13, 2008

A signal 16/23, ambulance requested, MVA, was toned out to the Smithtown Fire Department at 17:10:01 on Sunday April 13, 2008. The incident was reported on West Main Street and Route 25A.

 

Responding to the scene was Rescue 4-2-9, Engine 4-2-15 and ambulances 4-2-61 and 4-2-64. Upon arrival units found an accident involving two cars. One of the vehicles made contact with an exterior wall of the Oasis Bar.

 

Two injuries required the need for ambulance transport. One of the injured was transported to St. Catherine of Sienna and the other to the Stony Brook University Medical Center.

April 12, 2008

Fatal Smithtown MVA

A signal 16/23, ambulance requested, MVA overturn, was toned out to the Smithtown Fire Department at 01:49:18 on Saturday morning April 12, 2008. The incident was reported on Landing Avenue, just north of Edgewood.

 

Responding to the scene was Rescue 4-2-9, Engines 4-2-4, 4-2-15 and ambulance 4-2-61. Upon arrival units found a Ford Explorer resting on its driver side with the occupant pinned.  Heavy rescue efforts commenced. First responder 4-2-80 medically monitored the occupant.

 

At 02:10 a signal 50 DOA was relayed to dispatch.

 

At this time, 02:41, there has been no identification of the victim or cause of the accident. The Suffolk County Police Department is investigating.

 Update 12:20:00

The driver of the fatal accident has been identified as Paul Michele of Smithtown

 

Suffolk County Police said Michele left the roadway north of Edgewood Avenue, struck an embankment, and flipped.

 

 Suffolk PD said a formal investigation is still underway.

Motor Vehicle Accident Fatality in Smithtown

 At 04:10:58 on April 10, 2008 a signal 16-23 ambulance requested/motor vehicle accident/heavy rescue was toned out to 10 East Main Street and the corner of Elm Avenue.  Chief of Department Thomas Duckham (4-2-30) arrived on the scene to find a vehicle fully involved in flames on the eastbound side of Main Street.

The impact of the crash toppled a LIPA pole a few feet west of where the vehicle stopped. The pole was also on fire.

Responding to the scene were Engines 4-2-1, 4-2-4 and 4-2-5.

Chief Duckham requested a hand line be employed to extinguish the flames. The initial attack moved forward, and then was halted until LIPA arrived on the scene to shut down power.

Power was shut down and the knock down of the fire once again commenced. The fire was fully extinguished and units were up.

At this time at least one confirmed fatality has been reported.

Power has been shut down to sections of Smithtown as a result of the accident.

No cause for the crash has been determined. Suffolk County Police are investigating.

Motor Vehicle Accident/Overturn

A signal 16/23 MVA was toned out at 1449 (2:49PM) on March 16th. The one vehicle incident involved an overturn. The MVA was located on the eastbound lanes of Route 347, just east of Route 111.

The accident scene was under the command of 4-2-31 (Chief Duffy). Responding were Engines 4-2-2 and 4-2-4, Hook and Ladder 4-2-7, Fire Medic ambulances 4-2-61, 4-2-62 and 4-2-64.

The driver and passenger were removed from the overturned vehicle and transported by 4-2-61 and 4-2-62 to Stony Brook University Medical Center.

Units were up with the exception of 4-2-61 and 4-2-62, traveling to the hospital at 1517 (3:17PM).
