A signal 16/23, ambulance requested, MVA, heavy rescue was toned out to the Smithtown Fire Department at 19:56 (7:56 PM) on Tuesday, May 20, 2008. The incident was reported eastbound on Jericho Turnpike between Plymouth Blvd. and Meadow Road.
Responding to the scene were Chiefs 4-2-30, 4-2-31, 4-2-32, 4-2-33, Rescue 4-2-9, Engine 4-2-15 and Ambulances 4-2-61 and 4-2-62.
Upon arrival units started working on a two car head on collision involving a driver in each vehicle. One driver was able to leave the vehicle under his own power.
In order to remove the driver of the other vehicle, 4-2-30 requested heavy rescue. Spreader and a cutting tool were used to force the driver’s side door so the occupant could be removed.
Two injuries required the need for transport. 4-2-61 went to St. Catherine of Sienna Hospital where the driver of one of the vehicles sustained minor injuries.
Ambulance 4-2-62 transported the other driver to the Stop and Shop parking lot on Jericho Turnpike in Smithtown where a Suffolk County Police Helicopter was waiting to airlift the driver to the Stony Brook University Medical Center. There is no report on her condition at this time.

A fatality resulted in the two car collision. Mary Stone, 63, of Coram was pronounced dead after she was airlifted to Stony Brook University Hospital.
Amr Mohamed Nagy, 18, of Commack, who was transported to St. Catherine of Sienna, was treated and released.
